Many of our youth and Youth Group activities require parents to sign and submit a permission slip.  Please remember that you must have an up-to-date permission slip for each activity or the staff can't allow your child to participate. 

The links below will allow you to print out a permission slip so you may sign it and submit it to the Parish staff.   If you have access to a scanner, you may fill out and sign this form, scan and send it to the parish Secretary or Christian Education Director.
